Referral Form and Background Information
How can I get an assessment at Family Futures?
The majority of assessments are funded through the Adoption and Special Guardianship Support Fund (ASGSF). Alternatively, some families get funding approved through the child’s Education, Health and Care plan (EHCP). Family Futures is an Ofsted Outstanding provider of therapy services and we accept private referrals which are funded directly by families.
The family’s Social Worker will need to complete our Referral Form (self funders will need to complete the form independently) and email it to us at assessments@familyfutures.co.uk.
We then assess whether it’s important to have a multidisciplinary assessment or whether another route would be more appropriate – like a standalone Theraplay MIM assessment or Sensory Integration assessment, for example.
How long does it take to get an assessment date?
Once the referral form has been approved a full costing breakdown will be provided. Family Futures does not hold a waiting list but accepts referrals on a rolling basis. Assessments are scheduled approximately six weeks after funding is approved.
ASGSF funding
The ASGSF will only accept applications from a Social Worker. If you do not have a Social Worker, please contact your local Regional Adoption Agency or Local Authority’s Post Adoption/Special Guardianship team.
Other information you’ll need to provide
Family Futures will require a signed contract prior to an assessment being booked as well as all the relevant background information being received to fully inform the team prior to the assessment. This information will include:
Adoption:
- A copy of the Form E (CPR)
- A copy of Form F (PAR)
Special Guardianship:
- Viability Assessment
- Chronology Prior to SGO Placement
Foster Care/Other
- Chronology
- Care Plan
It is important to note that the recommendations may not be for the family to access services at Family Futures if there are alternative providers who can carry out therapeutic work or manage high risk situations sooner than Family Futures can.
